Refresher Module 1 (Interview)

Video Information:
This subject is 66 years old and her son is her collateral source who visits her several times a week. The interview was videotaped at 1:30 p.m. on Monday, December 21, 1992 at the Memory Diagnostic Center at Jewish Hospital in St. Louis by Dr. John Morris. The subject correctly identified her most recent job and the retirement date although it was unclear on the video.

This subject is 66 years old and her collateral source is her husband of 50 years. The subject was born on 3/21/1932 in Mathews, Missouri (in the Bootheel). The subject’s last school was Gideon School District in Gideon, Missouri. She completed the 8th grade. Her husband’s (collateral source) main occupation was a machinist for McDonnell-Douglas. He retired voluntarily in January of 1994. This interview was videotaped at Barnes-Jewish Hospital, St. Louis, Missouri on Friday, September 18, 1998.
