CDR® Certificate Request

The Knight ADRC attempts to retain copies of all certificates; however, this may not always be possible. It is the trainee’s responsibility to retain copies of their certificates at the time of issuance.

You can use the form below to request a copy of a previously issued CDR Certificate via the online training found on this website. This form is NOT for individuals trained in-person at the Knight ADRC or at an Investigator Meeting. If the requested certificate exists, it will be sent to the email you provide below. If no certificate exists, you will receive an email indicating such. Note that the Knight ADRC cannot provide certificates for trainings that were not certification-based, nor can certificates be provide for CDR training that was conducted by other institutions/companies.
