The Clinical Dementia Rating (CDR®) Assessment Protocol (often referred to as the CDR Worksheets) has been translated into multiple languages when can be viewed HERE as a PDF. In order to download any of the languages, you will need to license the CDR via our Office of Technology Management/Tech Transfer (OTM). If a language you need is not already available, OTM can help you secure the proper permissions to translate the worksheets.
