This subject is 64 years old and his collateral source is his sister who visits him monthly. This subject was born on March 29, 1929 in St. Louis. The last school he attended was Rankin Trade School in St. Louis where he completed a 2 year diploma past high school. His main occupation was as a machinist. The last major company he worked for was CPI, and he retired in 1990 due to problems with memory and thinking. The interview was videotaped at 2:00pm on Monday, April 12, 1993 at the Memory Diagnostic Center at Jewish Hospital in St. Louis by Dr. John Morris.
The collateral source reports on the videotape of ‘occasional’ incontinence for the subject. However, information obtained off camera as the collateral source was exiting the interview room was that the subject is routinely taken to the bathroom every two hours; otherwise, the CS thinks he would be frequently incontinent.
She additionally indicated that the ability to eat without much assistance during the Christmas meal was the exception rather than the rule; in the residence, he often is fed.
