Module 0 (Overview of the CDR)

This 20 minute video provides an overview of the CDR given by Dr. John C. Morris at Washington University in St. Louis.

Module 1 (Interview)

Video Information:
This subject is 66 years old and her son is her collateral source who visits her several times a week. The interview was videotaped at 1:30 p.m. on Monday, December 21, 1992 at the Memory Diagnostic Center at Jewish Hospital in St. Louis by Dr. John Morris. The subject correctly identified her most recent job and the retirement date although it was unclear on the video.

Video Information:
This subject is 61 years old and her collateral source is her husband of 18 years. This subject was born on November 1, 1931 in Blooming Rose, Missouri. The last school she attended was Licking High School in Licking, Missouri where she completed her high school diploma. Her main occupation was as mail room supervisor for a brokerage firm called Steiffel-Nicholas where she was laid off in 1988. She also worked for a friend who was a hairdresser for about a year. The interview was videotaped at 3:00pm on Monday, June 12, 1993 at the Department of Neurology at Jewish Hospital in St. Louis by Dr. John Morris.

Module 7

Video Information:
This subject is 80 years old and his collateral resource is his wife of over 50 years. The subject was born on 7/8/12 in Niles, Michigan. The last school he attended was Kalamozoo College in Michigan where he completed one semester. His main occupation was in business management as Head of Personnel at the A & P Tea Company. He retired in 1976 due to health problems and the company was reorganizing. The interview was videotaped at the Department of Neurology at Jewish Hospital in St. Louis by Dr. John Morris. For the purposes of scoring, the subject correctly remembers the date and time.

Video Information:
This subject is 75 years old (born 1/17/1920) and his spouse serves as his collateral source. The subject was born in St. Louis, Missouri and the last grade he completed was 12th at Cuba High School in Cuba, Missouri. His last major job was serving as a foreman at Anheuser-Busch until he retired in 1982 at a normal retirement age of 62. This interview was videotaped on October 6, 1995 at Jewish Hospital, St. Louis, MO by John C. Morris, M.D.
