CDR® Certificate Request

The Knight ADRC attempts to retain copies of all certificates; however, this may not always be possible. It is the trainee’s responsibility to retain copies of their certificates at the time of issuance.

You can use the form below to request a copy of a previously issued CDR Certificate via the online training found on this website. This form is NOT for individuals trained in-person at the Knight ADRC or at an Investigator Meeting.

If the requested certificate exists, it will be sent to the email you provide below within 24-48 business hours excluding US holidays.

If no certificate exists, you will receive an email indicating such.

If you do not receive an email with 48 business hours, check your spam and other folders. If you still don’t see an email from us, you have provided an invalid email address or your mail server is experiencing an issue. Return to this page and provide an email address from a different provider (Gmail, Yahoo, etc.).
